2012 Combined Registration/Medical Form (WBSL Registration/Medical_Form.pdf) - mandatory form that must be turned in for your child to be properly registered (with medical release info).
Volunteer Application Form (Volunteer_App_2012.pdf) - Fill out if you check "volunteer" on the registration form (team mom's, head coaches, or assistant coaches should complete and turn in this form).
Parent Code of Conduct Form (WBSL_Parent_Code_of_Conduct.pdf) - Complete & turn in at registration or to your child's head coach before the season begins.
WBSL Local Rules & Regulations for 2012 Season (WBSL Local Rules-Regs_2012.pdf) - Read, sign, & turn in to your child's head coach before the season begins.
