  • 2010 Player Registration Form (WTLL_Player_Registration_Form.pdf) -  mandatory form that must be turned in for your child to be properly registered.
  •  
  •  2010 Medical Release Form (WTLL_Medical_Release_Form.pdf)  - mandatory form that must be on file with WTLL before season practices start.
  •  
  • Volunteer Application Form (Volunteer_App_2010.pdf) - Fill out if you check "volunteer" on the registration form (team mom's, head coaches, or assistant coaches should complete and turn in this form).
  •  
  • Parent Code of Conduct Form (WTLL_Parent_Code_of_Conduct.pdf) - Complete & turn in at registration or to your child's head coach before the season begins.
  •  
  •  WTLL Local Rules & Regulations for 2009/2010 Season (WTLL Local Rules-Regs_2010.pdf) - Read, sign, & turn in to your child's head coach before the season begins.
